Fill out your health form

If you are unsure whether or not you need to enter an illness, injury or condition you have or have had, we recommend you do enter it. Then, the information is assessed on whether it is relevant or not.

If you have not been checked or treated for the disease in the last ten years, you do not need to provide health information that is more than ten years old.

Even if you have or have had an illness, you can still purchase insurance. The most important thing is that you fill out the health form and tell us about your illness so that we can determine what it means for your insurance. You may have to pay more for, or you may not be covered for this illness. Sometimes you may not be offered insurance.

It is easier to give you the right price and terms if you answer the questions in the form correctly and completely. If you answer incorrectly or leave something out, you may be unable to obtain insurance. It is also possible that you will not receive compensation even if you have insurance.

Insurance is trust. We believe what you tell us is true, both when you purchase and use the insurance. And on the day you need us, you should have confidence that you will receive the assistance you require.

You will be notified of any changes and why they have occurred. If you do not want the insurance with the new terms, you can cancel it and receive a refund of the money you paid. You must declare in writing that you do not want the insurance. You also get your money back if you are refused.

You must complete a health form in order to receive the correct insurance price and terms. It is a legal requirement to provide health information in a health declaration form.

Answer as accurately as you can. It is possible to provide additional information for most questions. You will be asked at the end of the form if you have any additional information to provide. Here you can point to questions that you did not answer well enough, and provide additional information.

Most people take between 10 and 15 minutes to complete the health form.

You know your health best. The GP does not always have all your health information. You may have had illnesses or ailments for which you did not see a doctor but which are relevant to the health assessment. You may also have received treatment elsewhere that your GP is unaware of.

According to the Insurance Contracts Act, you may only be asked for information from the previous ten years.

More information from a doctor or other practitioner is sometimes required. The reason could be that more information about one or more illnesses or injuries is required, or that it is preferable for the GP to provide an opinion because a large insurance amount has been requested. By law, such information cannot be obtained without your permission.

When you sign the health form, you give permission to the doctors, therapists and institutions listed on the form to obtain information from you. Information can also be obtained from NAV (the Norwegian Labour and Welfare Administration) and other insurance companies. It is only permitted to collect information that is required to conduct your health assessment.

If your child has lived in a country outside of Western Europe, the child must have undergone the Norwegian Paediatric Association's recommended examinations. If this is not done, you will not be able to obtain children's insurance until the child has lived in Norway for a year.

If you are adopted and do not know your family's medical history, you can answer "no" to the questions about your family's medical history. You will be asked at the end of the form if you have any additional information to provide. Here you can point to questions that you did not answer well enough, and provide additional information.
