Fill out your health form

When you purchase personal insurance, you are required by law to provide health information on a health declaration form.

When purchasing personal insurance, you will be asked to complete the statutory health certificate. The certificate is a form that only takes 10-15 minutes to complete and contains questions about your health, lifestyle and occupation. The information you provide in the form is used to calculate the best price and terms and conditions for you.

You are insured while your information is being processed

You are temporarily insured while your health information is processed once you have completed, signed, and submitted your health certificate. This means that you are covered for unforeseen events that are unrelated to your health at the time you sign the agreement.
